The answer to this question is why VNA of Central Florida, Inc./Community Care for the Elderly (VNACF/CCE) now offers professional care management services for a fee.
We understand the anxiety and fears associated with growing older. Our experienced eldercare professionals are licensed nurses and social workers who work very hard to preserve the independent lifestyle of the older adult. Our care managers provide a comprehensive assessment of the individual's needs, arrange for support services and when needed, offer a broad range of options for the individual and/or family to consider.
VNACF/CCE is also familiar with the resources available in the Central Florida community and works closely with a carefully screened network of home healthcare providers, personal care workers and others.
Our care managers visit the client in the home, assess their situation and make professional recommendations for the appropriate services to meet the client's needs. The client and his/her family will also receive information about specific agencies and facilities, their services, fees and staffing. If requested, care managers are available to help the client and family choose an agency or facility, arrange for the most appropriate services, provide follow-up and ongoing consultation.
